That is some good anatomy
The face of the thing is very cool
The smudges over the image make it look tainted and dirty which adds to the cool
When I hear (read?) the word "poison" I always think of poisonous people, how relationships are soured and how some people just suck the life out of you.
I guess my interpretation was along the lines of that, the relationship between the two people is posionous, this picture is from her perspective. Even if he is a good person he is poison to her because the relationship is a negative thing in her life.
